What is Shoulder Impingement Syndrome?

How is the shoulder structured?



The shoulder is the most flexible joint in our body, which makes its structure a bit complicated. But don’t worry, I’ll break it down for you!

1.Shoulder Blade (Scapula)
This is the flat bone at the back of your shoulder. On the top part, there is a bony projection called the acromion, which is often the main cause of shoulder impingement syndrome.

2.Arm Bone (Humerus)
This is the bone of your upper arm. It connects with the shoulder blade to allow free movement of your arm.


3.Rotator Cuff
These are the muscles and tendons that help move and stabilize your shoulder. Without them, lifting or rotating your arm would be difficult.

4.Subacromial Space
This is the small space between the acromion and the rotator cuff. When this space becomes narrow, the bones and tendons can rub against each other, causing pain.

Why does shoulder impingement syndrome happen?



Shoulder impingement happens when the subacromial space becomes too narrow, leading to the rotator cuff and bursa (a small fluid-filled sac) being compressed or irritated. Here are some common reasons:

1.Bone Shape Issues
If the acromion is not flat and is shaped like a hook, it can easily press on the rotator cuff.

2.Posture Problems
If your shoulders are hunched forward or your shoulder muscles are weak, it can cause misalignment of the shoulder bones.

3.Repetitive Movements
Frequently lifting your arms (e.g., during sports, housework, or work) puts a lot of strain on your shoulder.

What are the symptoms of shoulder impingement syndrome?



•Pain when lifting your arm!
If you feel pain, especially when lifting your arm between 60 to 120 degrees, it might be shoulder impingement syndrome. This is called the “Arc of Pain.”


•Shoulder pain at night
The pain often gets worse at night, making it hard to sleep.

•Difficulty moving your arm
The pain can limit your ability to move your shoulder freely. You might also feel strong resistance at certain angles.

How is shoulder impingement diagnosed?



1.Simple Tests
Doctors may use tests like the Neer Test or Hawkins-Kennedy Test to check for shoulder impingement.

2.X-rays or MRI
X-rays can show the shape of the acromion, while MRIs can check the condition of the tendons and bursa for more accurate diagnosis.
